In order to make the information you gather on your pages pass to Mailchimp, please, follow this tutorial:

You will need to build a page by adding the input forms you need to it:

After that, if you have not done that, you will have to create a list in Mailchimp by clicking this button:

Provide all the information needed to create a list and proceed to configuring the merge tags for that list in the settings:

You will have to add all the fields you want to gather on your optin pages by clicking this button:

Select the type of the field to be "Text":

Each field you make should be set as visible and not required.

After that configure the merge tags for the fields you have created in your Mailchimp list with the ones you can look up here:

For instance, for the phone field, it will be PHONE, for the last name it will be LNAME and so on.

Once you are done configuring the list fields in Mailchimp, don't forget to save the changes.

The last step would be integrating Mailchimp list on your page.

After that the contact information will be passed to Mailchimp successfully, once somebody opts in.
